Use this form to apply for new registration with the College of Acupuncture of Prince Edward Island.
Please complete all sections clearly and submit the form to info@acupuncturepei.ca with your name followed by “Application for Membership” in the subject line.
The required application fee should be sent by e Transfer to payment@acupuncturepei.ca with your name and “Application Fee” in the eTransfer note.

All official documents (transcripts, certificates, diplomas, etc) from institutions must be sent directly to:

We will not accept copies sent by the applicant to the College
When submitting your application, please include:

• Proof of Identity (Driver’s License or Passport)

• CPR-C or BLS Certificate

• Professional Liability Insurance (minimum $2,000,000 per claim/occurrence, with aggregate limit not less than $2,000,000 excluding legal or court costs)

• Criminal Record Check (with Vulnerability Sector Check)

• Diploma(s) and/or Degree(s) (original sent directly from granting institutions to the College address at the top of this form)
• Full Transcript(s) from all relevant institutions (originals sent directly from the institutions to the College address at the top of this form)
• Certification Examination Certificate (sent directly from the certifying agency to the College address at the top of this form)
• Permit to Work in Canada (foreign applicants)
